Safety Protocol as of 4/30/2020
As a healthcare provider, the health of our patients, doctor ' and associates is our top priority as we reopen to service your needs. As a result. we wanted to take this time to remind you of the precautions we are taking to ensure the entire Goddard Family Eye Care community is safe.

We ask you,
If you are exhibiting signs of COVID 19 or if you believe you have been in contact with someone who may have been exposed, we ask you to reschedule your appointment.
Please come to your appointment by yourself or limit who comes with you to one person so we can follow CDC social distancing guidelines.
If you need someone to bring you to your appointment, we ask that they wait outside in their vehicle
Upon your arrival, if we cannot observe social distancing dl,le to other patients in our office, we ask that you wait outside and we will call you when we are ready to see you. Please be sure we have your correct cell phone number.

Steps We are Taking to Ensure your Safety
We will be practicing Social Distancing. We will be limiting the number of Patients we will see per hour in order to perform a thorough cleaning after each exam. We will be limiting the number of patients in our office at a time. If you will be picking up your glasses or contact lenses, or need an adjustment or repair, we ask that you call for a convenient time so we don't have a lot of patients in the office at one time.

Patient Welcome Forms

Patient Welcome forms are available on our website (www.goddardfamilyeyecare.com). Please be sure to coml)lete, print, and bring with you the day of your appointment. This will avoid using clipboards and pens in the office that could spread any virus.


Temperature Upon Arrival
We will be taking your temperature upon arrival with a contactless thermometer. If a fever id detected, we will ask you to reschedule your appointment.

Protective Equipment and Cleaning Supplies
Masks and gloves will be used at all times by Dr. Goddard and st aff . Patients are asked to maintain wearing their mask to the appointment. ll>isinfectant wipes and hand sanitizer will be available to all patients. We will be washing our hands for 20 seconds after every patient interaction.

Frame Selection Safety
You will be supplied a pair of gloves for your frame selection. Our staff will disinfect all frames after use. A sanitary bath will be available for you to place all frame you have tried on.

Contact Lens Exams and Dispensing
We will be postponing all new contact lens fittings at this time in order to minimize exposure risk.
